MCO- HEALTHFIRST Transition help & FAQs

Please refer to the member’s letter to effectively communicate with the Patient and to assist them.  Healthfirst will notify the member by February 1.   We are giving all advance notice to assist them in the transition.

To stay with Elara Caring and their aide they choose one of the following two options:

  1. A member can convert to a Healthfirst Dual or MLTC plan (Complete Care or Senior Health Partners).  Please note, you must meet eligibility requirements.  To do so, complete the CFEEC form (linked and also in section below) and that came with the letter and return to Elara Caring in the postage paid self-addressed stamped envelope or e-mail to support@elara.com. 
     

  2. A member can switch to another Medicaid Managed Care Plan.  To do so, they must call Medicaid Choice at 855-222-8350. Here, we will need to confirm if your Doctors are in the Plan (offer assistance). One may choose from the following:

 

  • Centerlight (PACE)

  • Emblem

  • Empire

  • Fidelis care

  • MetroPlus

  • United HealthCare

  • One may also choose to stay with your current Healthfirst Managed Care Plan however, you will need to select another vendor for you Home Care Services who will provide a you with a new Aide.

WHY? 

Healthfirst is limiting their members’ choice when selecting their own home care provider by terminating our participation in its Medicaid Managed Care network.  The move will save the Plan money. Elara will continue to provide services as a participating provider in HealthFirst’s dual (Complete Care) and long term care plans (Senior Health Partners).

When?

As of March 1, 2019, Elara will no longer be participating in HealthFirst’s Medicaid Managed care plan. Elara will continue to provide services as a participating provider in HealthFirst’s dual (Complete Care) and long term care plans (Senior Health Partners).

What Can I Do?

To keep an Elara Aide, take action immediately. One may apply for long term care benefits and switch to HealthFirst’s long term care plan, Senior Health Partners.  If a member has both Medicaid and Medicare, they may enroll in HealthFirst’s dual plan, Complete Care.
