• My health condition has recently gotten worse.
• My physician has placed me on a new medicine and/or changed my existing medicines.
• I have recently been in the hospital or been to the emergency room due to an illness.
• I have been recently discharged from the Nursing Home and need help at home.
• It is difficult for me to leave home to see my physician.
