Can your child benefit from in-home services?

If your child is under 21 years of age and at least one of the following apply, you may benefit from a free in-home assessment:

My child has been diagnosed with a long-term illness.
My child has a disability or special healthcare need.
My child is has recently been in the hospital.
My child has a new diagnosis or medical equipment.
My child has a long-term health condition that is getting worse.
My child has had an organ transplant or is on a transplant list.
My child requires medical equipment such as feeding tube or suction machine.
My child requires ongoing medical treatments such as injections, IV therapy, or oxygen.
My child has been diagnosed with a developmental delay.
I am concerned about my child’s growth and development.
