Jordan Health Services is licensed to provide skilled care services
to persons of various ages and medical conditions. These services are for patients who have
acute medical conditions that require short-term intervention by Licensed Nurses, Therapists,
Medical Social Workers and Home Health Aides.
Services Provided Include:
- Skilled Nursing
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Home Health Aides
- Medical Social Services
- Respiratory Screenings
Eligibility:
- Be homebound (Medicare and Medicaid patients only)
- Have a skilled medical need
- Be under a physician's care
Typical patients are persons with recent hospitalizations, new or exacerbated diagnosis, new or
changed medication and decrease in functional/mobility status. Our goal is to maximized the
patient's health in a short time and improve their long-term quality of life by educating them
and their caregivers about their disease process and/or functional disorder.
Our nurses are trained to assess each patient's condition and develop a patient-specific care
plan, under the direction of a physician. With the input of the patient, our team of nurses and
therapists work to meet the goals outlined in the patient's care plan.
