Jordan is excited to announce the first of its kind LIVING WELL clinical programs. “Living Well with Heart Failure” is a comprehensive home care program designed to improve the lives of those with Congestive Heart Failure. Congestive Heart Failure is the most common reason for hospitalization among adults over age 65. The HEART FAILURE program is designed to improve patient outcomes, while decreasing hospitalizations and/or visits to the Emergency Room.
Each patient who qualifies for this program will be provided with useful tools to improve the quality of life for those patients with heart failure, including scales to monitor sudden weight gain, a nutritional guide, device to monitor daily blood pressure, and additional educational information to be used by the nursing and therapy clinical team. During the program a comprehensive plan is developed with the patient, caregivers, and physician to meet the needs of the patient with heart failure.
