Jordan Health Services Announces New Living Well Program

Mount Vernon, Texas — April 20, 2010 — Jordan Health Services announced the newest addition to its Living Well disease management programs. Living Well with Diabetes is a comprehensive program designed to teach the diabetic patient how to manage this complicated disease.

“I am confident with the rollout of Jordan’s newest clinical program, we are going to be able to impact our patients in a very positive way and help them take control of their disease,” said Susan Stell, the Company’s Chief Operations Officer.

Patients Learn Vital Care Information
As a patient you will learn how to use Meds Meals and Moves; basic concept that teach patients the importance of medication management, carb counting and meal planning, as well as safe exercise. Jordan’s Living Well with Diabetes program will also prepare the patient for things like blood glucose monitoring, foot care, insulin administration, and more.

Diabetes Educator
Jordan is also excited to introduce Frankie Washburn, RN, CDE, COS-C. Frankie is a certified Diabetic Educator with 20 years experience in home care. She is just one part of a caring team of qualified clinicians at Jordan Health Services who are ready to assist in the management of diabetes.
