Jordan Health Services is licensed to provide skilled care services to persons of various
ages and medical conditions. These services are for patients who have acute medical conditions that require short-term
intervention by Licensed Nurses, Therapists, Medical Social Workers and Home Health Aides.
Medicaid
There are two State Medicaid Programs offered, Primary Home Care/Family Care and Community Based Alternatives. Primary Home Care (PHC) is a non-technical, medically-related personal care service.
The Community Based Alternatives program provides home and community based services to the elderly and adults with disabilities. Services are provided as a cost-effective alternative to institutional care facilities.
