Community Based Alternatives
The Community Based Alternatives program provides home and community based services
to the elderly and adults with disabilities. Services are provided as a cost-effective
alternative to institutional care facilities.
Services Provided Include:
- Nursing Service
- Adult Foster Care
- Assisted Living/Residential Care Services
- Adaptive Aids and Medical Supplies
- Minor Home Modifications
- Emergency Response Services
- Occupational Therapy
- Physical Therapy
- Respite Care
- Personal Assistance Services
- Speech Pathology
Community Based Alternatives Eligibility:
- Age
The individual must be age 21 or older.
- Financial Eligibility
The individual must be a Medicaid recipient or be determined eligible
for this program. This determination is based on the income and resource requirements
for Medicaid benefits in nursing facilities. The caseworker applies allowable exclusions to income and resources.
- Medical Necessity
The individual must meet the same medical necessity determination requirement as applicants for nursing facility care.
