If you (or someone you know) receive Medicaid and at least one of the following applies, you may benefit from a free in-home assessment:

I have a medical condition that limits my ability to perform tasks such as bathing or grooming.
My condition limits my ability to cook, wash my clothes, or clean my home.
I have recently been in the hospital or the emergency room due to an illness.
I have recently been discharged from the Nursing Home and need help at home.
I have little or no caregiver support during the day.
I have difficulty remembering when to take my medicines.
