• I need assistance with my bathing or dressing.
• My condition limits my ability to cook, wash clothes, or clean my home.
• I have recently been in the hospital or the emergency room.
• I have recently been discharged from the Nursing Home and need help at home.
• I have little or no caregiver support during the day.
• I have difficulty remembering when to take my medicines.
