<p class="justify">As a RN Case Manager (CM), you will direct and coordinate all Primary Home Care, Family Care, CBA, and special health care programs and services within an assigned territory. By Coordinating the care provided by all disciplines, you will directly impact the patient's outcomes. Your district will be armed with a team of RN Supervisors, Field Supervisors and Attendants, which will allow you to delegate the needs of the patient, physicians and referral sources to those who are best suited to handle the job. Jordan RN Case Managers carry hand-held PDAs which allow individuals quick access to their co-workers. Field clinicians begin and end their day from their home and no longer come into an office each day. Jordan provides opportunities for growth by providing free Continuing Education (CE) through Jordan University.</p>

The Director of Nurses (D.O.N.) directs and coordinates skilled nursing services, therapeutic services and/or primary care programs associated in rendering home health care that is under the direction of a physician. The D.O.N. plans and conducts in-services and discussions with the Administrator and clinical staff to encourage participation in formulating departmental policies, in promoting initiative, in problem solving and in the interpretation of regulations and procedures. The D.O.N. coordinates activities with the various agency departments, and promotes and maintains favorable relationships with the staff, the doctors and discharge planners, the patients/clients and their families, and the public.  The D.O.N. analyzes and evaluates all provided services and programs to improve coordination, quality and continuity of patient/client care and to plan more effective utilization of staff time and abilities.

    <li>Must be a registered nurse (RN) licensed in the state of Texas or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).</li>

    <li>Have at least one (1) year experience as an RN obtained within the last 36 months.</li>

<p class="justify">The Non-Nurse Supervisor is a strategic member of the JHS healthcare team supervising the delivery of non-nursing care and related activities as required by the various TDHS programs. The Supervisor is responsible for visiting Non-RN supervised clients in the client's home, hiring, training, supervising and evaluating Attendants; reporting and maintaining records; monitoring the client's status and is in constant communication with JHS staff, physicians, TDHS case workers, client's family members and other contract program representatives. In order excel in your position, a Field Supervisor should be a good planner and well organized, very intuitive, able to interact effectively. Included in a team of an RN Case Manager, RN Supervisors, additional Field Supervisors and Attendants, you will be well equipped to handle every aspect of your job.
