Cardiocom, the experts in telehealth, is a world class developer, manufacturer and clinical service provider of award-winning telehealth solutions.

With more than 10,000 clients throughout the state of Texas and growing, Jordan Health Services has expanded its chronic disease clinical pathway programs to ensure their patients are supported proactively with home based education, skilled nursing and therapy intervention to support their ability to age in place. The addition of telehealth to their transitional care program provides state of the art technology in tracking the daily vital signs of these patients electronically, coordinating proactive care intervention with hospitals and referring physicians, and reducing unnecessary hospitalizations and urgent care visits. The Commander Home Telehealth System guides the patient through an interactive “Health Check” that includes the collection of symptoms, care plan compliance, and vital signs such as weight, blood pressure, oxygen saturation and heart rate.
It is common knowledge that lean protein is healthy for diabetics. Turkey, in fact, is one of the types of meats that diabetics are recommended to eat. Not only is it a wonderful source of protein, but it is also very low in fat in comparison to any other type of meat. The one thing that those with diabetes should know, however, is that the turkey skin is not healthy.

Fill Up On Vegetables:

If you have diabetes, the first thing that you may want to consider doing when you sit down to the Thanksgiving table is fill your plate up with vegetables. Broccoli, green beans, asparagus, peas, carrots and leafy green salads are all vegetables that you should feel free to indulge in.

Stuffing Should Be Avoided:

Although stuffing is a food that is very traditional to Thanksgiving, it is probably something that you will want to avoid eating if you are diabetic. The main reason is because it is packed with carbohydrates. In about 1/3 cup of stuffing, you will find nearly 45 grams of carbohydrates.

Cranberry Sauce In Moderation:

Although cranberry sauce does contain a good amount of carbohydrates and sugars, you do not need to entirely avoid it on Thanksgiving if you are a diabetic. It is known to be pretty healthy, mainly because of the health benefits associated with cranberries. As long as you are able to limit yourself to only eating cranberry sauce in moderation, it is acceptable.

The Mashed Potatoes Issue:

Many diabetics are under the impression that they need to avoid mashed potatoes altogether. This is really not the case, however. Although potatoes are very high in carbohydrates, it is okay to eat them in moderation. Know that one cup of mashed potatoes will contain about 20 grams of carbohydrates. Also know that you are going to lessen the overall healthiness of mashed potatoes when you choose to add gravy or butter. Although it normally may be okay to do, Thanksgiving is a time when you will be eating a lot of unhealthy foods that you normally wouldn't eat. For this reason, you may want to stick to adding pepper, salt or sour cream to your mashed potatoes.

No Marshmallows Or Caramel:

Two ingredients that are very commonly used in Thanksgiving dishes are marshmallows and caramel. Sweet potatoes, for example, can be a very good alternative to traditional mashed potatoes for diabetics. However, most people add marshmallows on top of their sweet potato dishes. Any diabetic will want to avoid this because, although marshmallows are not entirely unhealthy, they are known to be high in sugar levels. Any dish that includes caramel is something else that you will want to avoid, as caramel is also known to be high in sugar. You may find caramel in certain vegetable dishes or in desserts.

Mount Vernon, Texas, October 31, 2012 — Jordan Health Services today announced that its Texarkana, Arkansas office has been named to the Top 500 of the 2012 HomeCare Elite™ and its Arlington, Sherman and San Antonio, Texas offices all have been included on the 2012 HomeCare Elite list, a compilation of the top-performing home health agencies in the United States. Now in its seventh year, the HomeCare Elite identifies the top 25 percent of agencies and further highlights the top 100 and top 500 agencies overall. Winners are ranked by an analysis of publicly available performance measures in quality outcomes, process measure implementation, patient experience (Home Health CAHPS), quality improvement, and financial performance.

“The 2012 HomeCare Elite winners demonstrate a commitment to providing patient-centered care and serving as leaders in the home health community. Their success offers data-driven proof of being well-managed and high quality care providers to hospitals, managed care organizations, ACOs, and other potential referral partners across the healthcare continuum,” said Susan L. Henricks, President and COO of National Research Corporation, the parent company of OCS HomeCare. “Again, this year, we updated our methodology to reflect the rapidly evolving quality-focused healthcare landscape and national value-based purchasing trends. We congratulate Jordan Health Services’ Texarkana, Arkansas agency on being one of the top 500 home care agencies in the country.”

“We are honored to be recognized as a Top 500 HomeCare Elite agency and to be included on the 2012 HomeCare Elite list,” said Dean Holland, CEO of Jordan Health Services, “thanks to our committed and dedicated staff who provide excellent care to the patients we serve.” He went on to say, “Jordan’s philosophy has remained simple through the years … home care is what we do, compassion and caring is who we are.”

<span class="justify“HomeCare Elite recognition continues to gain importance given the increased regulatory mandates and the threat of shrinking reimbursement revenue. Home health agencies that have earned recognition among the HomeCare Elite demonstrated that they not only can adapt to an evolving marketplace but continue to excel in clinical, patient experience, quality improvement, and financial outcomes,” said Marci Heydt, Product Manager for the post-acute care business group of DecisionHealth.

CIMA Hospice has been providing compassionate end-of-life care in South, Central and West Texas for the past six years and has seen positive growth and strong support throughout the communities they serve. In October of last year, Jordan Health Services and CIMA Hospice came together to provide a continuum of quality care by combining their community, skilled and hospice services. Since that merger, CIMA has opened their newest office in Tyler, TX and is expecting to open a subsequent location in Texarkana later this year.

Michael A. Perez, CIMA’s Director of Business Development explains, “We are creating a stronger company that will continue to provide quality, compassionate services to our patients and their families in their homes. We couldn't be happier that our organizations are joining forces, especially since our patient care values are so compatible.”

While CIMA is new to the East Texas area, CIMA is not new to hospice care. “It’s an honor to provide dignified palliative care to an area where it is needed. We are very blessed to have Jordan Health Services as a partner who cares greatly about their patients and the community. We’re excited to be a part of the team.” says Administrator, Amy Denning, RN, CHPN.

Jordan Health Services has been deeply rooted in East Texas for over three decades by providing skilled and community based services. Jordan’s offices in Tyler, Longview and Palestine will now be able to work in partnership with CIMA Hospice to provide quality end-of-life care.

The CIMA professional team includes a hospice medical director, hospice certified administrator, nursing staff, certified nurse aides, social workers and chaplains. CIMA Hospice began seeing its first patients in June of 2006 in San Antonio. Since then, CIMA has grown into the El Paso, Rio Grande Valley, Central Texas and East Texas regions.
